LASIK Eye Surgery - A Discussion About LASIK
Here you will find a discussion about the most important aspects to consider when contemplating LASIK eye surgery for vision correction. Obviously, in order to make an informed decision, you must understand what LASIK eye surgery is. LASIK is an acronym for “laser assisted in situ Keratomileusis”. Employing the use of a excimer laser, LASIK eye surgery is designed to correct nearsightedness (myopia), farsightedness (hyperopia), and astigmatism. The excimer laser is a “cool laser” and therefore it does not generate any heat. Before we examine the LASIK eye surgery procedure, we need to talk a little bit about eye anatomy.

The most important anatomical features of the human eye are the retina, lens, and cornea. The cornea’s basic function is to bend the light so that an inverted image is placed onto the retina. The back of the retina is made up of photoreceptor cells called rods and cones. When the photoreceptor cells receive the light, electrical impulses are transmitted along the optic nerve to the optical region of the brain. Within the optical region of the brain, the image is placed upright. While it is the cornea’s job to get the light rays to the photoreceptor cells on the back of the retina, it is the lens job to make sure these rays come to a sharp focus. In this sense, the lens can be considered a fine tuning mechanism, which results in the formation of a sharp image (usually) on the back of the retina where the photoreceptor cells reside.
In the case of the three most common eye problems, nearsightedness, farsightedness, and astigmatism, the cornea has a shape which is not normal. This results in the light not being focused normally like it should onto the back of the retina.
In nearsightedness, the cornea focuses the image in front of the retina. In farsightedness, the opposite occurs where the image is focused behind the retina. In astigmatism, the cornea’s shape is asymmetrical which results in light rays being focused at two points on the back of the retina, instead the usual one point of focus. So as you can see, in most cases, a reduction in visual acuity occurs because of the unusual cornea shape. Now, how does LASIK eye surgery solve this problem.
In LASIK eye surgery, the excimer laser alters the inter-molecular bonds within the cornea which results in giving the cornea it’s correct shape. When the cornea has a correct shape, the image can be focused onto the back of the retina. I know it sounds weird, back it works. To get into the molecular basis of how it works is outside the purview of this site. But from the perspective of the LASIK eye surgery candidate, it really works (most of the time), and this is your main concern. Lets look at the procedure employed for LASIK eye surgery.

1) Before the excimer laser can be used and is effective on the cornea, a thin layer of superficial tissue most be cut and pulled back to expose the cornea. This flap is created by using a motorized blade.
2) After the flap is pulled back to expose the cornea, excimer laser energy is applied to the cornea for a short period of time.
3) After excimer laser application, the flap is then placed back to it’s original position on the cornea.
4) This flap adheres without the need for sutures and acts as a band aid for the cornea. The bodies natural healing mechanism then takes over. The result is a cornea with the proper shape.
LASIK Eye Surgery - LASIK and LASEK Are Not the Same
LASIK and LASEK are actually slightly different procedures. The principle, as previously discussed, is basically the same for each. In LASEK, when the cornea flap is produced, a much thinner layer of tissue is cut. This epithelial tissue is ultra-thin. After excimer laser application to the cornea, this thin layer of tissue is put back in place, just like in LASIK eye surgery, and serves as a band aid for the healing process. For those individuals who have unusually thin tissue overlying the cornea, or have other eye anomalies, LASEK eye surgery is the preferred method.
LASIK Eye Surgery - LASIK and LASEK Risks

LASIK and LASEK surgery is not without risks. As in all surgical procedures, LASIK and LASEK do have some risk associated with the procedure. By having a highly experienced surgeon perform the procedure, the risks can be minimized. So what are the risks from a percentage perspective. In the late 1990’s, about 5% of individuals undergoing the procedure had some sort of complication. Currently, LASIK and LASEK surgeons are reporting about 1% of individuals developing some sort of complication. Other studies have indicated that the incidence of LASIK eye surgery - patients having unresolved complications six months after surgery has been estimated from 3% to 6%. Reported figures for safety and efficacy are open to interpretation. So what is regarded as a complication.
The following are the possible complications:
1) Unusual sensitivity to light.
2) Halos or glare. These conditions occur if light enters the eye around the edge of the treatment area. They can usually be corrected with an additional treatment.
3) To much correction or to little correction.
4) Unusual astigmatism. This can occur if the cornea heals in a wavy configuration.
5) Regression
6) Contrast Sensitivity
These are the possible complications. You should obviously talk to your surgeon about the possible complications. Statistically, most complications come from the use of the motorized blade for creation of the corneal tissue flap. The use of this blade can result in cutting anomalies, which results in complications. With the introduction of a new cutting procedure developed by the company IntraLase, the cutting anomalies have virtually been eliminated. Instead of the older motorized blade method, IntraLase develped a laser to cut the corneal flap. The draw back to using the Intralase laser is that it makes the procedure more expensive. Whether using the motorized blade method or the Intralase laser, most individuals who undergo LASIK or LASEK eye surgery do not develop complications.
LASIK Eye Surgery - Conclusion
The United States military is providing free LASIK eye surgery to some of it’s soldiers. Sports stars and movie stars have used this procedure extensively. For example, Tiger Woods, a top professional golfer, had the procedure done. But for Tiger Woods, the decision was easy. His “look” is worth millions of dollars. He has contracts with companies for which he endorses products. These companies require him to maintain a certain “look” or image. His recognizable face must remain recognizable. Imagine if the procedure did not work for Tiger Woods, and he ended up having the need for glasses. He would be able to continue playing professional golf, but he would lose millions in endorsement money. So for Tiger Woods, it was worth the risk.
For the past four years, Army officials at Fort Bragg, NC have been performing eye surgery, such as LASIK and PRK, on qualified soldiers. About 10,000 to 12,000 soldiers a year will not need to keep track of glasses, prescription goggles and contact lenses on the battlefield. For the soldier to fight properly he or she must be able to see the enemy. LASIK eye surgery also means they will never lose their glasses on the battlefield.
From a technological perspective, the procedure has developed to the point where there are fewer risks. As in all surgical procedures, the LASIK or LASEK candidate must weigh the benefits relative to the risks in order to make an informed decision. Given that the risks of LASEK or LASIK eye surgery are now considered acceptable by the U.S. Food and Drug Administration (FDA), for some individuals, LASEK or LASIK eye surgery provides a viable option for vision correction.
